to authorize Barbara A. Smith to resign as the personal representative of tha
estate of Gerald E. Smith Sr., deceased. Petitioner respectfully represents
that
1. On July 21, 2014, letters Administration were issued to petitioner, who
qualified and was appointed by the Register of Wills of the County of
Cumberland, Commonwealth of Pennsylvania, as administrator of the
estate of Gerald E. Smith, Sr., deceased. Petitioner thereafter acted
and is still acting as personal representative.
2. Gerald E. Smith, Sr., decedent named above, at the time of death
resided at 333 Ponderosa Road, in the city of Carlisle, County of
Cumberland, Commonwealth of Pennsylvania, and died on March 2,
2014.
3. The persons interested in the estate of Gerald E. Smith, Sr., deceased,
who do not join in this petition, and all other persons interested in the
estate of decedent, and their respective addresses are as follows:

4. Petitioner seeks to resign as such personal representative and to have
the Letters withdrawn based on her health. Her treating physician, Dr.
Michael DeMichele, advises that she is medically unstable from anxiety
and depression to continue as the personal representative of the
Estate. Dr. DeMichele's opinion is attached hereto.
Wherefore, Petition requests that the.Letters appointing her as the
Administratrix of the Estate of Gerald E. Smith, Sr., be withdrawn and she be
released from any further obligation a/AdmZin' r trix.

History of Present Illness:
This 68 year old female presents with:
1. anxiety
The patient presents with anxious/fearful thoughts,compulsive thoughts,decreased need for sleep,depressed mood,difficulty
concentrating,difficulty falling asleep,difficulty staying asleep,easily startled and excessive worry but denies fatigue.The patient's risk
factors include history of depression.The patient's risk factors exclude alcoholism,childhood abuse or neglect,chronic illness and family
history of anxiety.The anxiety is aggravated by conflict or stress but not with alcohol use.The patient had a fair response to drugs and did
not respond to alcohol.The anxiety is associated with sweating.The patient denies any nausea,urinary frequency,vomiting and weight
gain.Additional information:She took on executership of her husbands estate and is not medically able to handle to stress to the degree
that it is making her unable to function.
