designated therein. It is my intent that the last dated and
signed Memorandum shall control. If there is no Memorandum, it
is my intent that all of my tangible personal property shall be
and become a part of my residuary estate.
SECOND: I devise and bequeath all the rest, residue

Trustees, in their sole discretion, deem advisable. The Trustee
or Trustees, in exercising their discretionary authority with
respect to the payment of income or principal of the within Trust
to my grandchildren, shall take into consideration any income or
other resources available to my grandchildren from sources
outside this Trust.
Any income or principal not so applied shall be dis-
tributed to each grandchild when he or she attains the age of
twenty-three (23) years. In the event any of my grandchildren
die prior to the termination of the Trust established herein for
their benefit, the interest of my grandchild in said Trust shall
cease with any income and principal being divided evenly between
or among that deceased grandchild's brothers and sisters or the
separate trusts established hereunder for their benefit and, in
the absence of any brothers and sisters, to my other grandchil-

To transfer the account, we need each beneficiary of the decedent to complete an
enclosed transfer form. This form will provide the new registration information and
certify the Social Security numbers for each new account. This form will also let the
beneficiaries choose options for their new accounts.
Each beneficiary must sign their respective form in section three. If any beneficiary will
be adding any additional tenant to their respective accounts, we will require their
guaranteed signature on their form. All new account owners must sign their respective
form in section nine.
You can obtain a signature guarantee from an officer of a bank or trust company, or a
member firm of a United States stock exchange. A notary public cannot provide a
signature guarantee. In order for a signature guarantee to be valid, it must state the words
"Signature Guaranteed", be signed by the guarantor, and indicate his or her position and
title. It must also indicate the name ofthe institution issuing the guarantee.
As mentioned in our previous correspondence, we will also need an original state
inheritance tax waiver for the number of shares held in the account on the date of
decedent's death. The waiver must list the Fund name and the number of shares.
Post Office Box 2600, Valley Forge, Pennsylvania I94B2-26oo
(610) 660-1000 . www.vanguard.com
. .
The agency to contact to obtain the waiver varies from state to state. Common names for
this agency are the Department of Revenue, Comptroller of Public Accounts and State
Tax Commissioner. If you are not sure where to obtain the waiver, you should contact the
Registrar of Wills in your county.
We will transfer the account when we receive the requested documentation. For your
convenience, we have enclosed postage-paid envelopes.
If you have any questions or need further assistance, please contact a member of our
Transition Specialist Team at 1-888-237-9045. A dedicated Transition Specialist will be
pleased to assist you.

Dear Mr. Bogar:
We are sorry to learn of the death of Pauline E. Zedaker. The following information will
be helpful in settling the claim on this annuity policy. The value of the account as of the
date of death is $4,456.94. The policy was opened 11/07/1997 with an initial premium
of $4,055.46 and was invested in variable funds subject to the stock market. Pauline E.
Zedaker was the owner and annuitant of this policy.
In accordance with the policy's death benefits, the beneficiary(ies) may choose one of
the following options to settle the funds still accumulating:
1. Request that income begin immediately from any of the annuity income
options described on the enclosed list. This choice maximizes the
benefits of the policy since 100% of the annuity value is applied to
produce periodic annuity income. The guaranteed period of any option
selected cannot exceed the beneficiary's anticipated life expectancy.
Proof of birth is required if a life option is chosen. Benefit payments must
start within one year from the date of death.
2. Request a lump sum payment of the cash surrender value. This value is
paid in one lump sum; it would include all applicable surrender fees. This
payout must be made within 5 years from the date of the participant's
death.
Since this account was purchased with qualified (before tax) funds, we would suggest
that the benefiCiary take an annuity payout in order to spread the tax liability over the
payout period.
Because the policy is a legal document, certain forms need to be completed and
returned before settlement of the claim can be processed. Listed below are the
requirements to settle this claim.

1. A certified copy of the death certificate.
2. The original policy must be returned for lump sum settlement. If it cannot
be located, section G on the claim form must be completed, signed and
witnessed.
3. The Policy Claims Settlement Form. Please complete section A. A
settlement option must be selected as well as an election regarding
Federal Income Tax withholding. Please note that section F does not
apply to the settlement of this claim.
