1. The petitioner is Frances Starr Adams Zynski, an adult individual currently residing
and has resided for the previous five years at 232 Winding Way, Camp Hill,
Cumberland County, Pennsylvania.
2. Petitioner is requesting.this Honorable Court order her name be changed from
Frances Starr Adams Zynski to Starr Frances Zynski.
3. Petitioner is requesting this Honorable Court permit the change of her name because
she has been known as Starr Frances Zynski for the majority of her life and wishes
her legal name to reflect the name by which she is known.

WHEREFORE,it is respectfully requested this Honorable Court grant the
Petitioner's Petition for Changes of name, and enter an order directing that Petitioner's
name be changed from Frances Starr Adams Zynski to Starr Frances Zynski.

NOW, this o �, atni$12014, it is HEREBY ORDERED a 1/2 hour
hearing on Petitioner's Petition for Change of Name is scheduled for , the

of the Cumberland County Courthouse, Carlisle, PA 17013.
IT IS FURTHER ORDERED Petitioner shall prepare and advertise a notice of
filing of the petition for Change of Name in compliance with Rule 5 of the Cumberland
County Rules of Court and the 54 Pa.C.S.A. §701.
IT IS FURTHER ORDERED Petitioner shall appear before the Pennsylvania State
Police and permit them to obtain copies of her fingerprints for purposes of determining
whether or not the Petitioner has any prior criminal history. Upon receipt of a copy of the
petition and the Petitioner's fingerprints, the State Police are directed to determine the
existence of any criminal history of the Petitioner, and thereafter follow the requirements set
forth in 54 Pa.C.S.A. §702. The costs of the determination shall be borne by the Petitioner.

In accordance with the Name Change Act of 1998, the fingerprint cards of the above named individual, who
has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:

( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).

( ) A criminal record was revealed that might prohibit a name change. Because the subsection(s) of the

You are also advised that the above response is based on comparison of fingerprints and associated requester -
furnished information against a name index and fingerprints contained in the files of the Pennsylvania State Police
Central Repository - only - and does not preclude the existence of other criminal records which may be obtained in the
repositories of other local, state, or federal criminal justice agencies.
