To Whom It May Concern:
Per my discussion with Anastasia DiBartolomeo from the PA Department of Revenue, I am forwarding
payment in the tax amount of$223.51 to you regarding the Joint Bank Account Onlv which was open at
M&T Bank at the time of my mother's death. Mrs. Cipolla resided in Augusta, Georgia at the time of her
death. The Joint Bank Account was the only"asset" at the time of her death. We do have a Georgia
death certificate. No estate was open in Georgia or in Pennsylvania.
I was advised by Ms. DiBartolomeo that no Inheritance Tax Return was required. Per her instructions, I
am forwarding check in the amount of$223.51 to you in settlement of this matter.
