� n temt�rC`�`2, 1011 Judge Ebert appointed Cumberland County Agin and

w �om�unity�ices as David Steigleman's Plenary Guardian of Person and Estate. At
`� the �e of the�ppointment, Mr. Steigleman was in the Cumberland County Prison. The
next day he was admitted to the Claremont Nursing and Rehabilitation Center. The
administration was concerned by David's history of agitation, abuse towards his spouse,
and mental health issues. A meeting was held with two social workers and two Aging
staff and David. David was convinced that he could leave the nursing home and it was
his belief that the only reason he couldn't was because his wife had a "retainer" out on
him.
Upon admission, he was diagnosed with Alcohol induced dementia, HTN, Bi-
polar, PVD, Dementia with psychotic features, hyperlipidemia, and shoulder pain.
During this year he was also diagnosed with Debility, Chronic Obstructive Pulmonary
Disease and anxiety.
While his wife visited him in the Home, he made it difficult for her because he
was constantly begging her to take him home. It was explained many times that Aging
had been appointed his guardian by the court and it was our decision that he stay there,
but he did not seem to comprehend this. By the end of October, David began to settle
down so a Behavioral Case Review by a Psychiatric Nurse was cancelled.
In November physical therapy was ordered for 3 times a week for 30 days for
stretching and increasing the strength of his right shoulder. It was also ordered he get
double portions at meal time as he seemed to be hungry a lot of the time.
In January he was started on a course of anti-biotic for treatment of an upper
respiratory infection. The State Police stopped by the Home to retrieve his license. This
had been in the safe in the business office and was turned over to them.
In March he was given Occupational Therapy for self-care 4 times a week as he
had had a decline in his ability to do self-care.
Around May he again began to focus his attention on getting out of Claremont.
His wife became upset because that was all he would focus on when she visited him:
Another meeting was held with Aging staff, the Director of Nursing, 2 sociat services
and the Administrator. Based on an examination by a behavioral health and a psych
evaluation the belief that his continued focus on leaving was a result of inental health
issues and not dementia.
At the September care plan it was reported that the Home is allowing him the
freedom to go off the floor without supervision. In October, Aging staff took him and

another man for whom we are guardian out to lunch to Bonanza which he enjoyed very
much.
He attends different activities including outings, church services, VA parties and
likes to hand out with a few men on the floor. Medically he is stable and doing well.
It is the request of the Cumberland County Office of Aging that the Agency retain
Guardianship of Person for David Steigleman. The Claremont Nursing and
Rehabilitation Center has demonstrated an adequate quality of care for his medical
problems. We believe, therefore, that David Steigleman should continue to reside in that
nursing facility.
ESTATE
Cumberland County did not become Mr. Steigleman's representative payee for
his Social Security until October 31, 2012. Prior to this, his spouse, Trudi Steigleman
continued to handle his income. The agency did apply for medical assistance for David
which was approved beginning September 2011. MA determined that all of his income,
with the exception of insurance and Medicare premiums, will go to his wife under
spousal impoverishment.
His car had been sitting in Philadelphia for 2 years and was not worth much. It
was donated to the Purple Heart organization. The Agency signed on his behalf.
During the process for MA recertification in September 2012, it was discovered
that his spouse's Social Security of$800 a month was not included in the MA formula.
The Home claimed she had never given them her information and she claims she did.
This changes the formula and the difference has to be paid to the home. It was agreed
that this would be done on a monthly basis.
It is the request of the Cumberland County Office of Aging that the Agency retain
Guardianship of Estate for David Steigleman.
