� � ' rOn �ie�e�r�ber 12, 1011 Judge Ebert appointed Cumberland County Aging and
e�cC�Oomr�nity S�w+�es as David Steigleman's Plenary Guardian of Person and Estate. At
;tf4� tir� of th��pointment, Mr. Steigleman was a in the Cumberland County Prison.
�T�Fe n�t day he�as admitted to the Claremont Nursing and Rehabilitation Center
where`�i'e continues to reside.
Upon admission, he was diagnosed with Alcohol induced dementia, HTN, Bi-
polar, PVD, dementia with psychotic features, hyperlipidemia, and shoulder pain. Since
then he has been diagnosed with debility, Chronic Obstructive Pulmonary Disease,
anxiety, GERD and Restless Leg Syndrome. This year he was diagnosed with
cataracts, weakness, ambulation dysfunction, asthma, and plantar fasciitis.
David's overall appearance is better and he now has a showering schedule that
he adheres to. He received an electric razor as well as sports related garments for
Christmas which helps with his appearance.
In April his wife took him to Philadelphia to attend his brother's funeral service.
He stayed overnight at his son's house.
In May Aging staff again took David and the other resident out to Hoss's to
celebrate Dave's birthday. He also received a Phillies shirt for a present.
Around June he started to complain of pain and weakness in his legs. Physical
Therapy has been ordered for strengthening as well as assessing for a wheeled walker.
Staff reports that he is doing better mentally. He seems more content, enjoys activities
and has adjusted to nursing home life rather well over the last year. He has attended
Home activities that took place outside the Home. He has also been going out of the
facility several times with a friend. This has been approved by Aging. The gentleman
always calls Aging ahead of time who sets up the visit.
David has begun to express the desire to move to the Philadelphia area.
Aging. Staff explained that an appropriate facility would have to be found that would be
willing to admit him. The other issue would be the legal action necessary to transfer the
guardianship. An agency that did guardianships in the Philadelphia area would have to
be found.
David expressed an interest in having an occasional beer so his doctor wrote an
order that he could have 12 oz. of beer once a day upon request.

David was visited by Aging staff 7 times this year, including when he was taken
out to eat.
It is the request of the Cumberland County Office of Aging that the Agency retain
Guardianship of Person for David Steigleman. The Claremont Nursing and
Rehabilitation Center has demonstrated an adequate quality of care for his medical
problems. We believe, therefore, that David Steigleman should continue to reside in that
nursing facility.
ESTATE
The amount of insurance premium on the SKF Pension decreased from $336.34
to $163. 95. When Aging made an inquiry they were told that the spouse had been
incorrectly flagged as disabled. The pension plan agreed to not bill for the next 8
months in order to make up the error.
Since the arears with Claremont was paid up, in September, Aging was paid for
the guardianship fees it has not received for 11 months. From that point on, Aging will
receive $100 each month.
He currently receives $1340 in Social Security.
It is the request of the Cumberland County Office of Aging that the Agency retain
Guardianship of Estate for David Steigleman. Attached is a financial summary of David
Steigleman's income and expenses. Also attached is a spread sheet representing his
checking account register.
