PETITION TO OPEN AND AMEND PROBATE RECORD
PETITIONER,through her Counsel, Johnson Duffle Stewart & Weidner, requests that the Probate
Record be opened and amended and in support states the following:
1. Romaine M. Neidigh, the Executrix named in the Will of the Decedent, filed a Petition for
Probate and Grant of Letters on December 10, 2014.
2. Letters Testamentary were granted to Romaine M. Neidigh.
3. On Petitioner's presentation of a short certificate to M&T Bank to convert Decedent's account
into an estate account, M&T refused to recognize Petitioner's authority as Executrix based on the fact that
Petitioner had an account with the Bank under the name of Romaine V. Neidigh.
4. Romaine M. Neidigh and is also known as Romaine V. Neidigh, but she is one and the same
person.
WHEREFORE, Petitioner respectfully requests that the Register of Wills open and amend the
Probate Record to reflect that Petitioner is also known as Romaine V. Neidigh.

1, Romaine M. Neidigh, also known as Romaine V. Neidigh, hereby confirm that the statements in the
foregoing Petition to Open and Amend Probate Record are true and correct to the best of my knowledge,
information and belief.
I understand that any false statements herein are made subject to penalties of 19 Pa. C.S. Section
4904,relating to unsworn falsification to authorities.
