Mary Cavanagh was admitted on 8/30/04 and covered by Geisinger.
She was discharged on 9/5/04 to Holy Spirit Hospital and was not a bed hold.
Mary Cavanagh was readmitted on 9/16/04 and Chris Cavanagh signed the admission
agreement.
Chris Cavanagh failed to complete a Medical Assistance application after several
attempts were made, resulting in an outstanding balance to West Shore Health & Rehab.
