PERSON
On June 20, 2005, Permanent Guardianship of Person and Estate for Theresa Ceja
was granted by Judge Kevin Hess to Cumberland County Office of Aging. Prior to this
appointment Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4
Camp Hill, Pennsylvania. She was admitted to the Forest Park Health Center Carlisle
on May 27, 2005 after the outcome of an Emergency Guardianship hearing.
Mrs. Ceja continued to reside at Forest Park Health Center until her death. She was
moved from the dementia unit to the nursing unit in August 2007. Due to her severe
cognitive loss she was no longer benefiting from the activities and structure of the
dementia unit. She continued to be diagnosed with senile dementia Alzheimer's type,
depression, Parkinson's, PVD, osteoporosis, hypertension and anxiety. She also had
two "unstagable" open areas, one on her two and one on her face.
She was nonverbal and as well as non-ambulatory for over a year. The staff at Forest
Park Health Center provided maximum hands on assistance with activities of daily
living. Theresa was a 2 person assist with the Hoyer lift for all transfers. Because she
was a high risk for skin breakdown she had a pressure relief cushion on her wheel
chair. Hospice services from Heartland Hospice began in November 2013.
In December 2014, Theresa started to decline. At the Care Plan Meeting in early
December it was reported that she was medically stable and that the nursing home
continued to monitor and treat the wounds on her foot and face. Per the Doctor notes
they were both "unstagable." They had discontinued debriding and were treating them
with calcium dressings. The foot wound at its largest measured 4.5 centimeters and
was reduced to 3.5 centimeters. They also reported that her appetite had begun to
decrease. They continued to use splints for comfort. In December she also received a
special Broda chair for easier repositioning and greater comfort.
On December 23 we received word from the Hospice nurse that Theresa was declining,
not eating, and her gag reflex was diminished. They put her on comfort meds every 6
hours as needed. It was a liquid morphine that was placed under her tongue. We
received word the next day that she had passed away. Causes of death listed on the
death certificate are sepsis, aspiration pneumonia, dysphagia, Alzheimer's disease.

Burial was at Woodlawn Gardens in Harrisburg, PA. A graveside service was held with
2 Aging staff and her grandson in attendance.
ESTATE
Pre-funeral arrangements had been made with Hollinger Funeral Home in Mt. Holly.
Mrs. Ceja had an irrevocable burial trust with Sovereign Bank. In addition, $2000 was
placed in a life insurance policy with the funeral home as the beneficiary.
The agency had received word earlier this year that there were monies from
Metropolitan Life Insurance that had been from Theresa's mother's life insurance policy
for which Theresa was the beneficiary. These monies were in Pennsylvania's
Department of the Treasury's Unclaimed Property. Our agency staff has provided the
different documentation that was requested in the process of trying to claim this money
for Theresa. If this money totaling $1667.82 is ever received, it will be deposited into her
bank account.
The final balance in her banking account was $4,215.77. The required report to the
Department of Public Welfare's Estate Recovery was made. A final summary of
income and expenses and check register are attached.
