consideration of the attached Petitio� Under §5511 of the Probate, Estates and Fiduciaries Code
to Adjudge Joseph A. Nell to be Totally Incapacitated and to Appoint a Guazdian for His Estate
and His Person, it is hereby Ordered and Decreed that:
1. A Citation is awarded, directed to Joseph A. Nell to show cause why he should

of his person and his es[ate should not be appointed; the hearing thereon to be held in cour[room

2. Petitioner shall ca�se to be served (by personal service) the Citation with Notice
and Petition, pursuant to lhe provisioos of 20 Pa. C.S. §S51 !(a), upon the alleged incapacitated
perso� at least twenty (20) days prior to the court hearing for the Permanent Plenary
Guardianslrip. The conte�ts and terms of the Citation with Notice and Petition shall be read and
explained to the maximum extent possible in language and terms the alleged incapacita[ed
person is most Iikely to uuderstand, iu accordance with the provisions of 20 Pa. C.S. §551](a).
M affidavit of service contaiuing specific ave�men[s of the above requicements shall be
presented at the beginning of the cour[hearing.
3. At least tv✓enty (20) days prior notice of the court hearing together with a copy of
the Petition shall be given personally, by U.S. First Class Mail, Certified Mail, or by FedEx
Overnigh[ or 2-Day to all persons who are sui juris, reside within the Commonwealth of
Pennsylvania, and who would be entitled to ari iotestate shaze in the esta[e of the alleged
incapaci[ated person, to [he insti[ution providing residential serviees to the alleged incapacita[ed
persoq and to , in accordance with 20 Pa.C.S. § 5511(a).
An affidavit of service co�taining specific averments of the above requirements shall be
prese�ted at the beginning of the wurt hearing.
4. The a0eged iacapacitated person shall be present a[ [he court hearing unless it is
established by cleaz and eouvincing medical evideuce that his physica] or mental co�dition
would be harmed by his presence in court in accordance with the provisions of 20 Pa. C.S.
§SSll(a).
