COOLEY, : IN THE COURT OF COMMON PLEAS OF Deceased : CUMBERLAND COUNTY, PENNSYLVANIA ORPHANS' COURT DIVISION NO. 21-14-1048 PETITION FOR ORDER TO OPEN DECEDENT'S SAFETY DEPOSIT BOX Petitioner DAVID A. COOLEY,by and through his attorney, Susan J. Hartman, sets forth the following: 1. Carol A. Cooley, decedent herein, died testate on October 23, 2014, a resident of Cumberland County, Pennsylvania. 2. Her Last Will and Testament was probated and Letters Testamentary were issued to David A. Cooley on November 4, 2014 and docketed to File No. 21-14-1048. 3. At the time of her death, Carol A. Cooley was possessed of Safe Deposit Box Number 3775 located at M&T Bank, 1 West High Street, Carlisle, Pennsylvania. 4. The aforesaid Safe Deposit Box was held jointly with her husband Dennis who predeceased her on May 26, 2014. 5. David A. Cooley, Executor of the Estate of Carol A. Cooley, resides in the State of Texas and is unable to come to Pennsylvania in order to inventory the contents of the Safe Deposit Box. 6. M&T Bank requires a Court Order designating a representative other than the N Executor of the Estate to inventory a Safe Deposit Box. 6. David A. Cooley wishes to designate the Attorney for the Estate of Carol A. Cooley, Susan J. Hartman, Esquire, as his representative for the purpose of completing the inventory, removing the contents and closing the account. Wherefore,the Petitioner prays this Honorable Court issue an Order authorizing Susan J. Hartman, Esquire, as attorney for the Estate of Carol A. Cooley to Inventory Safe Deposit Box 3775,to remove the contents and take possession of same, and to close the Safe Deposit Box Account. Respectfully submitted, usan J.
