PETITION FOR MODIFICATION OF COURT ORDER
AND NOW comes the Petitioner, Michelle L. Mansell,by and through her
attorney, Mark T. Silliker, Esquire, and respectfully requests this Honorable Court grant her a
modified Order or Court, and in furtherance thereto avers the following:
1. A Petition to Amend Record of Birth was filed with this Honorable Court on
May 8, 2014.
2. On May 9, 2014,the Honorable Christylee L. Peck granted the relief requested
in the Petition and directed that"... the Birth Certificate of the subject minor child, Elise Cynthia
Meck,born April 17, 2009, shall be amended so that heretofore henceforth the child shall be
known as Elise Cynthia Mansell, and the child's father shall be listed as Charles D. Mansell."
3. Unfortunately,the Pennsylvania Department of Vital Records will not
recognize or honor the Order of Court dated May 9, 2015. Personnel from the Department of
Vital Records have advised the undersigned,Attorney Mark T. Silliker,that in order to amend
the Birth Certificate of the subject minor child the Order must read as follows,to wit, "It is
hereby directed that the Birth Certificate of the subject minor child,Elise Cynthia Meck,born
April 17, 2009, shall be amended so that heretofore henceforth the child shall be known as Elise

Cynthia Mansell, and that Dean Howard McNaughton shall be removed from the child's Birth
Certificate as the father of the subject minor child, and Charles D. Mansell shall hereinafter be
listed on the Birth Certificate as the father of the subject minor child."
WHEREFORE Petitioner requests this Honorable Court issue a Modified Order
removing Dean Howard McNaughton as the father of the subject minor child, declaring
Charles D. Mansell the father of the subject minor child, and changing the child's name to Elise
Cynthia Mansell.
