Such personal service was made on February 17, 2015 at 4:22 P.M. at the following location: 59 West Louther Street, Carlisle, PA 17013 The undersigned further certifies that on February 13, 2015, a true and correct copy of the petition was served upon Bayada Nurses as an interested party who provides caregiver services via U.S. Postal Service Certified Mail. A return receipt was received from the U.S. Postal Service indicating that the interested party accepted the Certified Mail, and the online tracking indicated said mail was accepted on February 17, 2015.
