the Petition of Michelle L. Mansell, it is hereby ORDERED AND DECREED as follows:
1. The Order of this Honorable Court dated May 9, 2014 is hereby amended as
set forth hereinafter.
2. The Petition for Modification of Court Order is hereby granted.
3. It is hereby directed that the Birth Certificate of the subject minor child, Elise
Cynthia Meck, born April 17, 2009, shall be amended so that heretofore henceforth the child
shall be known as Elise Cynthia Mansell, and that Dean Howard McNaughton shall be removed
from the child's Birth Certificate as the father of the subject minor child, and Charles D. Mansell
shall hereinafter be listed on the Birth Certificate as the father of the subject minor child.
