PERSON
On April 15, 2010 Judge Ebert appointed the Cumberland County Aging and
Community Services Plenary Guardian of Person and Estate for William James Hill. Mr.
Hill was a resident of Manor Care Health Services-Carlisle at the time of the hearing
and continues to reside there. Mr. Hill had not been receiving adequate from the
residence where he had been staying prior to his admission to the nursing home.
At the time of the Guardianship, Mr. Hill was diagnosed with severe malnutrition, urinary
tract infection, acute renal failure, deconditioning, hypertension, chronic anemia and
dementia.
Mr. Hill was wheelchair bound but was very good at maneuvering around the halls. He
was incontinent of urine and wore a catheter and would occasionally have urinary tract
infections from it. In November 2011, he was admitted to Holy Spirit Hospital for a
severe back pain which turned out to be the result of a severe urinary tract infection. He
was treated with antibiotics for 4 days.
The main problem had been Bill falling out of bed. He had a remote for his N but he
reach for things such as the TV remote or his shoes. He also began falling in general.
Around March 2012 Bill started to experience a decline in his health. He was very tired
most of the time and had lost his appetite. Bill had been placed on Hospice to give him
extra support and to encourage him to eat.
During the next to last visit with Bill, Aging staff reported that he would get extremely
tired and was somewhat anxious. According to the Hospice Chaplain, the Home was
having trouble getting him to eat. He didn't like the thickened liquid diet they had him on
and he did not like to be fed but usually he wouldn't feed himself. They were trying
different things to get him to eat more consistentty.

During the last visit, Bill was actually attending an activity this morning which he had not
been doing for a least a month. According to nursing home staff he had perked up
noticeably during the last three days. He had eaten his meals in the dining room with
his friends and had attended the coffee socials where he spent a little time talking with
some of the other residents. He passed away on July 16, 2012
ESTATE
After the guardianship appointment, the agency opened a guardianship checking
account at M8T Bank. The agency applied for and became his Social Security
representative payee.
At the time of his death, his Social Security was $642. His Room and Board at Manor
Care varies according to the number of days in the month but the last payment to Manor
Care was $529.02. They later refunded $179.28.
The last check for the Cumberland County Aging and Community Services for a
guardianship fee as allowed by Medical Assistance and approved by the Guardianship
Court Order was for$32.00 for Y: of a month. The balance in his account as of his
death on 7/16/13 was $338.42.
Attached is a financial summary of income and expenses. Also attached is a spread
sheet representing his checking account register.
