Specific Authority to Purchase "Flower" Bonds. To purchase United States Treasury"flower"bonds on my behalf and to borrow money as provided above for the purchase of such bonds. 8. Specific Financial Powers Defined by Statute. 1 The following powers are granted pursuant to Chapter 56 of the Pennsylvania Probate Estates and Fiduciaries Code as further defined therein: Page-5- 3 A. The following powers are granted pursuant to Chapter 56 of the Pennsylvania Probate, Estates, and Fiduciaries Code, as further defined therein: A. To authorize my admission to a medical, nursing, residential, personal care, or similar facility and to enter into agreements for my care in those facilities, or through a home care agency. My Agent may review medical records, and execute releases of confidential information from medical providers and insurers,or other third-party payers, and consult with my physicians and other health care personnel and providers,and shall be considered my personal representative for health care disclosure under the Health Insurance Portability and Accountability Act of 1996 (Pub. L. 104-191) (45 CFR Section 160-164), including, without limitation, designation of my Agent as a personal representative as defined in 45 CFR Section 160.105.
