PETITION FOR APPOINTMENT OF GUARDIAN
FOR THE ESTATE OF THOMAS E. MITCHELL
NOW COMES the Petitioner, Cheryl D. Mitchell, the mother
of the minor, Thomas E. Mitchell, who requests this Court
appoint a guardian of the estate of the minor person, Thomas E.
Mitchell, and in support thereof avers as follows :
1 . Petitioner is Cheryl D. Mitchell, who is an adult
individual residing at 545 Brandt Avenue, Apartment C, New
Cumberland, Pennsylvania 17070 .
2 . Thomas E. Mitchell is a minor residing with his
mother, the Petitioner, Cheryl D. Mitchell, at 545 Brandt
Avenue, Apartment C, New Cumberland, Pennsylvania 17070 .
Thomas E. Mitchell was born on February 1, 2000 .
3 . The relationship between the Petitioner and the minor,
Thomas E. Mitchell, is that of mother and child.
4 . An appointment of the guardian of the estate of the
minor is necessary because the minor is named as beneficiary of
Prudential Insurance Company of America Control Number G-95468
Claim Number 11529922 having a value of approximately
$15, 000 . 00 by reason of the death of Charles J. Mitchell, his
father, who died February 7, 2015 .
5 . The name and address of the purposed guardian is as
follows : Cheryl D. Mitchell, 545 Brandt Avenue, Apartment C,
New Cumberland, Pennsylvania 17070 . Cheryl D. Mitchell is the
mother of the minor child.
6. No notice is required to be given to the United States
Veterans Administration or its successor.
7 . The minor, who is the subject of this petition, is
under the age of eighteen (18) years of age and is therefore
not the petitioner herein.
8 . Cheryl D. Mitchell consents to this petition as a Co-
Admini:stratrix of the Estate of Charles J. Mitchell .

9;. Christine N. Shultz consents to this petition as a Co-
Admini'stratrix of the Estate of Charles J. Mitchell .
WHEREFORE, Petitioner requests this honorable court to
appoint Cheryl D. Mitchell as guardian of the estate of the
minor person Thomas E. Mitchell .
