AND NO W,this�day of�, 2015, based upon the evidence received and the

record, this Couri finds, by clear aud comincing evide�ce that Steve E. Kintz is adjudged a totally
incapacitated person.
The Coutt finds that Steve E. Kintz suffers from dementia, a condition that totally impai�s his
capacity to receive and evaluate information effectively,to make and communicate decisions
conceming management of his financial affairs, and to meet essential requirements for his physica]
health and safety.
Good News Consultiug, Inc. is hercby appoi�ted Plenary Peananent Guazdian of the PERSON
of Steve E. Kintz. The Guacdian of[he PF.RSON shall file a Report on[he soeial, medical and other
eelevant conditions as required by 20 Pa.C.S.A. § 5521(c)(ii) within the first twelve months of thei�
appointment and annually thereafter.
Good News Consulting, Inc. is hereby appointed Plenary Permanen[ Gunrdian of the ESTATF.
of Stevc E. Kintz. Bond is waived. The Guardian of the ESTATE shall 61e an inventory within Ihree
mouths of this decree of the incapacitated person's rea] and personal property, pursuant to 20
Pa.C.S.A. § 5521(b). In addition, the Guardian of the ESTATG shall file a report within the first
twelve months of theic appointment a�d at least annually thcrcafter, eontai�ing the information
required in20 Pa.C.S.A. § 5521(c)(1)(i).

� Neither the Guardian of the PERSON nor the Guardian of the BSTATE shall have authority to
enter a safery deposit box in the name of Steve E. Kintz (individually or jointly) without written Court
authorization.
The Guardian of the PERSON shall have authority and responsibility to decide where Steve E.
Kintz shall live and how meals, personal care, transportation and recreation will be provided. The
Guardian of the PERSON shall also have thc authority to authorize and consent to medica.l treatment
and surgical procedures necessary for the well-being of Stevc E. Kintz. 'I�he Guazdian of the PERSON
and the Guardian of the ESTATE shall coopera[e to piepaze a budget to eover the eost of pcoviding the
aforementioned services to the pecson otSteve E. Kintz.
The Guardian of the ESTATE shall have authority and responsibility to manage and use the
incapacitated pecson's property for his benefit in accordance with 20 Pa.C.S.A. § 5536(a). The
Guardian of the E3CA"fE is permitted to spend INCOME for the incapaci[a[ed person without the
CourPs written approval. Good News Consulting Ine. shall be entitled to reasonable fees for services
provided to the incapacitated person, ineWding a start-up fec of$499.00. If the incapaeitated person
qualifies for Medical Assistance benefits, Good News Consultiug Inc. shall also be enlitled to $100.00
per month or other amount established by the Commonwealth.
Banks and other financial institutions shall release to the Guazdian of the ESTATE all
information related ro Steve E. Kintz upo�the Goardian's request.
Good News Consulting, Inc. having now been appointed Guardian of the ESTATE of Steve E.
Kintz, and having determined it appropriate to exercise the authority granted under 20 Pa. C.S.A. §
5604(c)Q) of the PEE Code to rescind any and al] du�able powers of attomey heremfore executed by
the incapacitated person, said power(s)are hereby deemed to be null and void.
� The aforementioned judicial determinations have taken into consideration the matters required
by 20 Pa.C.S.A. § 5512.1. The Court's findings of fact and conclusions of law have been piaced on the
record at the evidentiary hearing.
Steve E. Kintz is advised and informed that he has a right to appeal this determination to the
Superior Court of the Commonwealth of Pennsylvania and, further, may petition this Court at any time
to modify oi terminatc thc guardia�ship.
