FINAL ORDER OF COURT APPOINTING PLENARY CO-GUARDIANS

AND NOW, this �3 day of v'� � , 2015, a hearing in this case having been
held on Apri113,2015 and it appearing to the Court that REBECCA M. MAURER was served with a
Citation and Notice of this hearing on March 10,2015,and the Court finds finds from the testimony:
1. That REBECCA M. MAURER suffers from Downs Syndrome, a condition which
totally impairs her capacity to receive and evaluate information effectively and to make and
communicate decisions concerning her management of financial affairs or to meet essential
requirements for her physical health and safety.
2. That there are insufficient supports available to assist REBECCA M. MAURER in
such decisions and that there exists no other less restrictive alternative mechanism for decision-
making.
3. That based on the total incapacity of REBECCA 1VI.MAURER to receive and evaluate
information and to make or communicate decisions,plenary Co-Civardians of the Person and plenary
Co-Guardians of the Estate are required on a permanent basis.
NOW, THEREFORE, based on the clear and convincing evidence supporting the foregoing
findings it is ORDERED, ADJUDGED and DECREED that REBECCA M. MAURER be and is
hereby adjudged a totally incapacitated person and Maria S. Maurer and/or Randall C.Maurer and/or

Jessica C. Maurer are appointed Plenary Permanent Co-Guardians of the Person and Estate. As
Plenary Permanent Co-Guardians of the Person, Maria S. Maurer and/or Randall C. Maurer and/or
Jessica C. Maurer, acting jointly and/or individually, each have the authority to access all
REBECCA M. MAURER's medical records, including but not limited to psychiatric records,and to
make all decisions regarding medical treatment and life support. As Plenary Permanent Co-Guardians
of the Estate,Maria S.Maurer and/or Randall C. Maurer and/or Jessica C. Maurer will be responsible
for managing REBECCA M. MAURER's financial affairs in their capacity as Co-Guardians, as set
forth under the Probate,Estates and Fiduciary Code,20 Pa. C.S. §5501 et seq. The Co-Guardians will
have all powers stated thereunder as are necessary to manage RFBECCA M. MAURER's personal
and financial a�fairs, and specifically the Co-Guardians, acting jointly or individually, will have the
power to manage REBECCA M. MAURER's bank accounts located at Members 15� Federal Credit
Union and the power to manage any and all life insurance policies,any and all securities,mutual fund
accounts, certificates of deposits, including cashing out or withdrawing any funds.
The Co-Guardians will further have the power to prepare, execute and file any tax return with
the Internal Revenue Service on behalf of REBECCA M.MAURER and to request any information on
behalf of REBECCA M. MAURER from the Internal Revenue Service.
An Inventory must be filed within ninety (90) days. A report by the Co-Guardians shall be
filed within 12 months and annually thereafter.
REBECCA M.MAURER,an incapacitated person,has the right to appeal this Order of Court
by filing exceptions within ten (10) days of this date or to petition this Court for a review hearing to
modify or terminate the guardianship herein established.
The requirement of bond is hereby waived. BY TI-� OURT:
