Your Petitioner, John D. Perkey is a competent adult residing at X28-PinerGrove Cf' D o Road, Gardners, Pennsylvania, and is the Executor of the Estate of Ronald C. Wolfe;. —0 ` r- 2. Ronald C. Wolfe, Decedent, died on December 5, 2014 at the age of 77 y(I s, but o prior thereto lived and was domiciled at 36 Green Mountain Road, Gardners, Cumberland County, Pennsylvania. A copy of Decedent's Death Certificate is attached hereto as Exhibit «A» 3. Decedent died with a Will. Letters Testamentary were granted to John D. Perkey on December 17, 2014. A copy of Decedent's Last Will and Testament dated, November 25, 2014, is attached hereto as Exhibit "B." 4. Decedent's wife, Betty J. Wolfe, predeceased Decedent on November 18, 2014 at the age of 82 years, but prior thereto lived and was domiciled at 36 Green Mountain Road, Gardners, Cumberland County, Pennsylvania. Death Certificate of Betty J. Wolfe is attached hereto as Exhibit"C." 5. Betty J. Wolfe died without a Will. No letters were issued. 6. In an effort to gather all of the assets of Decedent, it has been determined that Decedent's spouse held an account with M&T Bank individually, with a balance of$4,705.32. A copy of the M&T Bank statement is attached hereto as Exhibit"D." 1 7. Decedent is the sole beneficiary of Betty J. Wolfe. There is no inheritance tax due to the Commonwealth of Pennsylvania. 8. Your Petitioner avers that there are no creditors of Betty J. Wolfe and no claims unpaid. 9. A paid funeral bill from Hollinger Funeral Home & Crematory, Inc. for Betty J. Wolfe is attached hereto as Exhibit "E." 10. The funds from the M&T Bank account will be deposited into the estate account of Ronald C. Wolfe and paid according to the Last Will and Testament of Ronald C. Wolfe. WHEREFORE, your Petitioner respectfully requests that an Order be made directing M&T Bank to close the account of Betty J. Wolfe, Deceased, and distribute the account balance to the Estate of Ronald C. WOLFE, of Cumberland County, Pennsylvania, declare this to be my Last Will and Testament and hereby revoke all prior Wills and Codicils. -- - 1: - --I-direct that-ail-my just debts, funeral expenses, administrative expenses, and any inheritance tax which may be assessed shall be paid from my estate as soon as practicable after my death. It is my wish that upon my death, my body shall be cremated, and half of my ashes shall be buried at Victory Church Cemetery, Gardners, Pennsylvania, and half of my ashes shall be spread at my home at 36 Green Mountain Road, Gardners, PA 17324. 2. 1 direct that all real property and all personal property that I own at the time of my death shall be given, devised, and bequeathed in accordance with the following: a) My niece, Donna Thomas, and my niece, Linda Russell, shall be given all of my antiques and glassware, in equal shares,per capita. b) The rest, residue and remainder of my estate shall be sold, and the proceeds shall be distributed in accordance with the following: i) Thirty-percent (30%) shall be given to Saint Paul Evangelical Lutheran Church, Carlisle, Pennsylvania. ii) Twenty-percent(20%) shall be given to Boys Town Organization, 14100 Crawford Street, Boys Town,Nebraska 68010. iii) Twenty-percent(20%) shall be given to St. Jude Children's Research Hospital, 262 Danny Thomas P1, Memphis, Tennessee 38105. iv) Ten-percent (10%) shall be given to the Salvation Army of 0 Carlisle, Pennsylvania. With the exception of paragraph 2(a)and 2(b)(vi), all gifts shall be used for non-administrative purposes. If a named organization in paragraph 2 no longer exists, ---- -- - -then-an-organization that offers substantially similar services shall receive-the respective percentage,with the organization to be chosen by my personal representative. 4. I appoint my friend, John D. Perkey, as Executor of this my Last Will and Testament. In the event that John is deceased,unable or unwilling to serve or shall cease to serve for any reason whatsoever, then I nominate, constitute and appoint Adam R. Deluca,Esquire, as alternate Executor of this my Last Will and Testament. 5. The Executor or Executrix of this Will shall have the power to distribute my estate in cash or in kind, or partly in either. 6. I direct that no Executor or Executrix acting under this Will shall be required to enter bond in any jurisdiction. 7. I recommend that my Personal Representative retain the law firm of Allied Attorneys of Central Pennsylvania, L.L.C.,to probate my estate.
