Politzer-Hardy, A minor under the age of fourteen years No. 2005 - ~ ORDER AND NOW, this 2~-\t day of j/.\ f I ,1 ,2005, upon consideration of the within petition of Geneva B. Politzer, it is hereby ordered and decreed that F&M Trust Co., the corporate co-guardian of the estate of Michael D. Politzer-Hardy, is hereby authorized to expend principal from the minor's estate, without further order of court, in the amounts and for the purposes specified as follows: "I C.J a. The sum of $1 ,200 may be disbursed directly to Geneva B. Politzer at any time after June 1,2005, to defray the expenses of transportation, meals, and lodging, in connection with the minor's surgery at Boston Children's Hospital. Ms. Politzer shall thereafter present to the corporate co-guardian appropriate receipts as evidence that the said funds were properly applied to the said expenditures for the minor's benefit, which receipts shall be accepted by the corporate co-guardian as full acquittance for the funds so advanced. b. The following sums shall be disbursed by the corporate co-guardian directly to the provider or supplier in payment for the indicated goods or services: i. Up to $1,000 to purchase a "memory-foam" mattress pad and other durable medical equipment not covered by insurance. ii. $500 - $600, for repairs to the petitioner's 1985 Volvo, to adapt the rear hatch to accommodate the minor's wheelchair. 111. Up to $1,500 to pay for the construction of appropriate handicap- accessibility modifications to the minor's dwelling, including a wheelchair ramp. iv. $500 to the petitioner's attorneys, for fees incurred in preparing the petition for guardianship.
