AND NOW this day o , 2015, upon consideration of
the Petition of Joanne Cave as Administratrix of the Estate of Thomas J. Cave, Deceased, it is
hereby Ordered that:
1. Settlement of all claims against Conifer Realty arising from the injuries and
resulting death of Thomas Cave for a payment of $60,000.00 is approved.
2. The execution by Joanne Cave of the Release attached to the Petition is
approved.
3. Payment of fees to Tucker Arensberg, P.C. of$17,500.00 is approved.
4. Reimbursement of costs to Tucker Arensberg; P.C. of $467.66 is approved.
5. Payment of$20,000.00 to Medicare Advantage Plan as payment in full of its
subrogation claim is approved.
6. Payment to Joanne Cave of the net settlement proceeds of$22,032.34 with said
payment being made pursuant to the Wrongful Death Act is approved.
