Erin Leigh "Ebersole" Klein is an adult individual with principal residence at 116 Cumberland Drive, Carlisle, PA 17013. 2. Erin Leigh "Ebersole" Klein is the natural mother of Carter Klein, (Social Security Number XXX-XX-1881), born June 6, 2007. 3. Carter Klein resides with his mother at 116 Cumberland Drive, Carlisle, PA 17013. 4. Carter Klein was left$113,441.52 dollars in the form of an Estate Account with The Adirondack Trust Company, left to him by Clifford Scott Klein (hereinafter Clifford Klein). 5. Clifford Klein died on October 13, 2012. 6. Clifford Klein was the natural father of Carter Klein. 7. The interest of Carter Klein should be represented in this matter by his natural mother, Erin Leigh "Ebersole" Klein. 8. The monies received from Clifford Klein are presently being held in an Estate Account at The Adirondack Trust Company in Saratoga Springs, New York. 9. No other guardian has been previously appointed for Carter Klein. 10. Erin Leigh "Ebersole" Klein, upon consultation with her financial planner,wishes to have said proceeds deposited in a Savings Account with Pennsylvania State Employees Credit Union (PSECU), as Account number 8417812628. WHEREFORE, Petition Erin Leigh "Ebersole" Klein respectfully requests that she be appointed as guardian for Carter Klein, a minor,for purposes of investing the $113,441.52 left in an Estate 4coe&Rt with The Adirondack Trust Company, in accordance with thiition.
