NOTICE OF TESTIMONY BY TELEPHONE
This notice is submitted to this 1-16norable Court by counsel for Stephanie Kuhn, the
mother of the alleged incapacitated person, Brandon Kuhn. The Community Law Clinic hereby
submits, this Honorable Court should allow the testimony of Dr. Michael A. Fueyo, Dr. Vivian
C. Faircloth, Dr. Mario Sangillo, and Nicholas Fioreilo to be taken by telephone, regarding the
hearing for Adjudication of Incapacity and Appointment of a Guardian of Brandon Kuhn
scheduled July 6, 2015 at 2:00 p.m., for the following reasons:
1. Dr. Michael A. Fueyo is reasonably unable to testify in person due to employment.
2. Dr. Vivian C. Faircloth reasonably unable to testify in person due to employment.
3. Dr. Mario Sangillo reasonably unable to testify in person due to employment.
4. Nicholas Fiorello is located at least 50 miles from the location at which the tribunal
will conduct the hearing.
