ORDER APPOINTING JAMES A. U SH ES UIRE AS GUARDIAN OVER THE
ESTATE OF JA S L. ULSH A N1INOR
AND NOW, this CJ day of , 2015, upon consideration of the
Petition for Appointment of James A. Ulsh, Esquire Over the Estate of'James L. Ulsh, a Minor
filed in the above-captioned matter pursuant to 20 Pa. C.S. §5111 et sec�. and Pennsylvania
Orphans' Court Rule No. 12.5 it is hereby ORDERED as follows:
1. James A. Ulsh is hereby appointed guardian of the Estate of James L. Ulsh, a
minor.
2. Upon presentation of a copy of this Order to any financial institution, the above-
named guardian shall have authority to access any and all funds belonging to James L. Ulsh.
3. The above-named guardian shall have all rights, powers, and duties granted to
guardians over minors' estates as allowed by 1aw.
4. The above-named guardian is directed to file an inventory within ninety (90) days
of the date of this Order and in accordance with the provisions of 20 Pa. C.S. §5142. Said
inv�t�y sl�l be filed�uith the Clerk of the Orphans' Court.
