Court finds by clear and convincing evidence that Sarah Quimby is an incapacitated person who
cannot manage her personal or financial affairs.
It is hereby ORDERED and DECREED that Keystone Guardianship Services is
appointed Plenary Guardian of the Person and Estate of Sarah (�uimby with powers and duties
including, but not limited to the following:
1. To consent to medical treatment;
2. To provide for the continued care and placeme�lt oF Sarah Quimby, including to
consent to and authorize admission to a long-term care or other apprapriate care facility;
3. To oversee her care at any long-term care or othe�r facilit}';
4. To complete and sign any necessary insurance and �Iedical Assistance forms;
�. To ascertain the amount and e�tent af any assets of Sarah Quimby ar�d to u�e such
funds as necessary to fulfill the preceding duties; and
(. To provide for the continued care aYid weli-being of Sarah Quimby, all in
accordance with the provisions of 20 Pa. C.S.A. §5501 et seq. and any and all other applicable
statutory provisions and as limited therein.

'7. Keystone Guardianship Services shall be paid a fee of Seven Hundred Fifty and
00/100 Dollars ($750.00) as an initial guardianship start-up fee from the estate of Sarah
Quimby's resources. Further, a reasonable fee for ser��ices of the guardian may be made and
shall be payable from Sarah Quimby's estate. To the extent Sarah Quimby is a recipient of
Medical Assistance benefits from the Pennsylvania Department of Human Services, Keystone
Guardianship Services shall be entitled to a payment of$100.00 p�r month, in lieu of Keystone
Guardianship Services' hourly rate. Said $100.00 per manth shall increase or decrease
consistent with Department regulations.
It is further ORDERED that Keystone Guardianship Services shall serve without
additional insurance or bond, but Keystone Guardianship Services is required to maintain its
current level of insurance and bonding.
It is further ORDERED that Keystone Guardianship Service shall have the authority to
spend reasonable amounts from principal, not to exceed $5,500, which amount includes the sum
of $5,344.87 Ms. Quimby received pursuant to an insurance policy of which she was the
beneficiary, an amount which also renders Ms. Quimby ineligible for Medical Assistance, for the
care and maintenance of Sarah Quimby without Court approval.
It is further ORDERED that within sixty (60) days of the aeath of the incapacitated
person or an adjudication of capacity and modification of existing Orders, Keystone
Guardianship Services shall file a final Report with the Court in accurdance with 2U Pa.C.S.A.
§5521(c)(2).
