1. A hearing on the attached petition is schedulzd for G�uc�l�.�t/.3 �(s at �•.� P.M.
in Courtroom No. �, Cumberland Counry Courthouscy, 1 Courthouse Square, Cadisle,
Pennsvlvazua.
2. The Clerk shalt issue a citation upon the alleged incapacitated person to show eause why he
oc she should not be adjudged an incapacitated persou and why the Court shoidd not appoint
a guardian of his or her estate and/or a guardian of his or her person.
3. At Ieast 20 days prior to the hearing, perso�al service of the citation and copies of the petition
and this preliminary decree shall be made upon the alle�ed incapacitated person. The
contznt, and terms of the petition shall be esplained to thc alleged incapacitated person to the
maximum estent possible in languave and terms the alleged incapaeitated pecson is most
likely to understand. An afYidavit of service shall be flled betore the hearing or offered as an
eshibit at the beginning of the hearine.
4. At least 20 days prior to the heazing, notice of Ihe petition aud hearine shall be given to (i) all
persons residi�� withi�the Commomvealth who are sui juris and would be entided m �are �
in the esrate of the alleged incapacitated person if he or she died intestate a�at time, -�nj to m rn

Such notice shall be given by meaos of pe�sonal service or certified mail. An affidavit of service
shall be ftlzd bzfore the hearing or offered as an exhibit at the beginni�g of the heazing.

5. At least 7 days p�ioc to the hearing, the petitionec shall notify the Comt, in wxiting, if counsel
has i�ot been retained by or o� behalf of the alleged incapacitated persoa If wu�sel has not
been cetained by or on behalf of[he alleged incapacitated persoq sueh notice shall contain
a�y pertinent information that may indicate the Court should appoint counsel to represent the
alleged incapacitated person.
6. The alleged incapacitated pe�son shall be p�esent at the hearing unless (i) the Court is
satisfied, upon the deposition or swom statement by a physician or licensed psychologist,
that his or her physical or mental condition would be haRned by his or her presence, or(ii) it
is impossible for him or her to be present because of his o�her absence from the
Commonwealth.
