AND NOW, this��ay of ��� , 2015, upon
consideratio� of the Petition by Cumberla�d QLunry Agmg & Community Services for
Appointme�t of�Plenary Guardians of the Pe�son and Estate of Jean L. Milsop and the hearing
held oo the PetiNon before this Court, the Court finds, based upon cicar and convincing cvidence,
that Jca�i L. Milsop is an incapacitated person in that she suffers from mental impairment that
prevents- hcr from �eceiving and evaluating information effectively arid prevents hcr from
tormuleting and communicating decisions to such a significant extend that she is unable to
ma�age her finencial affairs and meet the essential requirements fo� her physical health and
safety on a daity basis. Accocdingly, Cumbcdand County Aging & Communify Services is
hereby appointed as plcnary guardians of the person and estate of Sean L. Milsop.
Cumbedand County Agi�g & Communiry Services shall servc without bond and is
permitted to receive eompensation for its services as plenary guardians i� the amoont of$100 per
month or ten percent (l0%) ofjean L. Milsop's gross monthly income, whicheve� is less.
