The name of the minor for whose esta.te a guardian is requested is Azalea Stine. The minor is seven(7)months of age and was born on December 5, 2014. The minor resides at 905 Valley Street,Enola, Cumberland County, Pennsylvania 17025. 2. The minor does have a living parent. The minor's mother, Ta_ylor Lowe, resides at 905 Valley Street,Enola, Cumberland County, Pennsylvania 17025. The minor's father,Randall L. Stine, deceased, died on,May l, 2015, and he was not appointed a guardian of the estate of the minor by will, deed, or other instrument. 3. This petition is filed by the following person or persons: a. Taylor Lowe, residing at 905 Valley Street,Enola, Cwnberland County, Pennsylvania 17025,the minor's mother; and b. Janet Lowe, residing at 905 Valley Street,Enola, Cumberland County, Pennsylvania 17025,the minor's maternal grandinother. 4. The petition has the consent of the following person or persons, and the consent of each person is attached: a. Taylor Lowe,the minor's mother. b. Janet Lowe,the minor's maternal grandmother. 5. The minor has no guardian of her estate,and the appointment of a guardian for her estate is necessary because the minor is entitled to receive the death benefits from Hartford Life and Accident Insurance Co. policy number OGL402284 on the life of her father with an amount of $67,038.92. ..:„.r,1�.PI�.I....�,.�..11�1��. � 6. The name and address of each proposed guardian,the age af any proposed individual guardian, and the relationship of any proposed individual guardian to ihe minor, if any, are as follows, and the consent of each proposed guardian is attached: a.
