DIRECT PAYMENT OF INSURANCE BENEFITS OWED TO MINOR
ORDER OF COURT
AND NOW, this 30th day of July 2015, upon consideration of the Petition
Pursuant to 20 Pa. C.S. §5101(3) of Robin S. Jones to Direct Payment of Insurance
Benefits Owed to Minor, and with no response from respondent, it is hereby ORDERED
that Colonial Life & Accident Insurance Company shall issue payment of the benefits
owed to Jonathan M. Jones under the certain Life Insurance Policy on the life of Mark L.
Jones, now deceased, directly to Robin S. Jones and shall be mailed to her at 9
Mooredale Road, Carlisle, PA 17015. Petitioner's attorney's fees shall be payable from
the subject insurance benefits. Petitioner shall effectuate service of this Order and shall
file proof of service
