Attached is an Inheritance Tax Return for the Estate of Lila M. Mit hell, whoged'6n�
October 28, 2014.
The main asset of the estate is an annuity with Glenbrook Life Annuity, with a value of
$4,600.00. The beneficiary of the annuity is Martha M. Fitz, a friend of the Decedent. It is my
understanding that she was also the beneficiary of a life insurance policy held by the Decedent.
There were no significant probate assets and therefore no ability to otherwise pay the
inheritance tax obligations. We are therefore requesting that a separate billing for the inheritance
tax be issued to the beneficiary of the annuity. Her contact information is as follows:
