CONSENT OF INDIVIDUAL TO APPOINTMENT OF GUARDIAN
1, Henry H. Stanley, hereby consent that Louann Speese-Stanley be named the primary
guardian of the person and primary guardian of the estate of Diana Stanley, an alleged
incapacitated person, and I certify that:
1. I am a competent person of 18 years of age or older.
2. My interest in the above-named proceeding is as the biological father of Diana Stanley,
the alleged incapacitated person.
3. 1 reside at Lot 35 Salem Church Road, Mechanicsburg, PA 17050 .
4. 1 speak, read, and write the English language.
5. 1 am a citizen of the United States.
6. 1 am aware of my right to seek legal representation and have decided to proceed without
legal representation.
7. 1 understand that the Disability Law Clinic represents Petitioner Louann Speese-Stanley
in this action.
8. 1 fully understand the effects of granting primary guardianship of the person and the
estate to Louann Speese-Stanely.
