ANDNOW, this ��{� dayof �Jc�J.ST , 2015, ahearing �
iu this case having been held on �'�_, 2015, and i[appearing to the Court
that Mazy Martha Hagan was served with a Citation a�d Notice of this hearing on
Q a� �`1 , 2015, the Co�rt finds the following from the testimony:
1. The alleged incapacitated person, Mary Martha Hagan, suffered from dementia
and Alzheimer's.This conditio�has totally impaired hei capacity to receive and
evaluate information effectively to make and communicated decisions
concerning the management of her fi¢aucial affairs. This condition totally
impairs her capacity to meet essen[ial requicements for her physical health and
safeTy.
2. That there are insufficient supports available to assist Mary Martha Hagan in
oveccoming such limitations and that there exists no less resaictive alternative
mechanism for decision making other[han the appointment of a plenary
guardian of the person acid a plenary guardian of Ihe estate.

3. That based on the incapaciry of Mary Martha Hagan to receive and evaluate
infonnation and to make or communicate decisions, a plenary guardian of the
person and a plenary guardian of the estate are required on a permanent basis.
NOW, 7I3EREFORE, based on the cleaz and convinciog evidence
supporting the foregoing findings, IT 1S ORDERED, ADJUDGED AND DECREED that
Mary Martha Hagan be and is hereby adjudged a rotally incapacitated person a�d that her
brother-in-]aw, James Alber[ Hagan is appointed Plenary Permanent Guazdian of the
Person and Plenary Permanent Guardian of the Estate.
James Albert Hagan being the plenary guardian of the person, shall ttave plenary
powers over the person of Mary Martha Hagaz�, including but not 1'united to,full authoriTy to
consent to the general care, maintenance and custody of Mary Martha Hagan; the authoriry
to designate a place for Mary Martha Hagan [o live, including admission into a nursing
home; and[he authority to provide required consents or approvals on behalf of Mary Martha
Hagan which aze necessary for the well-being of her person. James Albert Hagan shall
assure that Mary Martha Hagan receives appropriate services and shall, if reasonably
possible, assist her in developing self-celiance and independence.
Jarnes Albert Hagan,being[he plenary guardiazi of the esta[e, shall have plenary
poweis over the estate of Mary Maztha Hagan, includiug but not limited to the aothorization
to access [he financial records of Mary Martha Hagan whethcr those records be at a Ba�k,
a Credi[Union, a Brokerage firm or elsewhcre and also the au[horization to spend income
$ ��1 �v � �� per month of the priucipal assets of Mary Marcha
Hagan.
As guardian, on behalf of Mary Martha Hagan, James Albert Hagan shall have
the authority [o engage in transactions with the Intemal Revenue Service and the
Pennsylvania DeparUnent of Revenue. These transactions shall include, but no[ be limited
to, filing Fedecal Income Tax Retums, 51ing Pennsylvania lncome Ta�c Retums and
requesting copies of pceviously filed taac retums.
If income or principal funds of Mary Mazfha Hagan are available or become
available, the guardian is au[horized to establish a prepaid irrevocable bu�ial account for
Mary Mar[ha Hagan up ro[he current medical assistance allowable amoun[.
Guardian shall not Vaosfer, sell, encumbei or cause any realty oi tiUe thereto
cur�ently owned by Mary Martha Hagan to become encumbered oi affected in a�y
manner unless they have first made application to this Cour[ and subsequently have
received the express written approval therefrom.
Uuazdian shall not be required to post bond.
The Powec of Attomey document appoin[ing James E. Hagan as agent foc Mary
Mar[ha Hagan is hereby revoked.
If there is a safe deposit box in the name of Mary Martha Hagan alone or in the
names of Mary Martha Hagan and another or others, said safe deposit box shall not be
entered by the guardian except in[he presence of a representative of the financial institution
where the box is located or in the presence of a representative of the Orphans' Cour[
Division. If the safe deposit box is jointly owned,five(5)days' notice of the proposed entry
shal] be given to the other owners by the guazdian. The representative present at the time of
entry shall make or cause to be made a mcord of Mary Martha Hagan's property, and said
rewrd shall be filed with the Clerk of the Orphans' Court Division. None of Mary Martha
Hagan's proper[y may be removed until after the aforesaid inventory is comple[ed.
An Inventory must be filed by the guazdian within ninety(90)days. A report by
each guardian shal]be filed within ninety(90)days and annually thereafter.
Mary Mar[ha Hagan, an incapacitated person, has the right [o appeal [his Order
of Cour[by Hling exceptions wiUuu ten (10) days of this date or to pe[ition this Cowt for a
review hearing to modify or termina[e the guardianship herein established.
