AND NOW, this `/� day of Se� �-.,.-Lv . 2015, upon a hearing where medical and
other testimony and evidence were presented, az�d in consideration of the within joint motion of
the Parties, the Coun makes the following findi�gs, pursuant [0 20 Pa. C.S. §5512.1:
FINDINGS:
1. Marie A. Jarusewski (the"RcspondenP') exhibits, at times, a "minimal -" or"mild
cognitive impairment."
2. The Responde�Ps mild cognitive impairment occasionally limits her capaciTy to make
decisio�s in he� best interests; her abiliry to communicate is highly-developed and is u�impaired.
3. The Respondent will benefit from a limited guazdianship, as hereinafter set forth, due to
her habit of revoking powers of atromey granted by her to her various children, zs they falt out of
herfavor.
4. Although the Respondent exhibits, at times, a mild cognitive impairment, she is not
totalty ineapacitated and thus a limited guacdianship of her person and estate is appropriate.
NOW THBREFORE, IT IS HERBBY ORDERED:
The petitione�s, Maureen F. Clay and Scoll Scott D. Jarusewski, are appointed Co-
Guardians of the person and estate of Marie A Jarusewski. The Co-Guardians' authoriTy shall
be ]imited to the foliowing powers, and shall be subject to thc obligations set forth herwndec and
in Section §5521 ofTit1e20:
a. Guacdinns shall have the authociTy to designate where the Respondent shall reside.
b. Guardians shall have custody of the RespondenT s bank account at Orrstown Bank,
subject to the corresponding duty to arrange for the pzyme�t of all of the Respondent's bills,
including her Marriott Visa bills and the expenses related m the RespondenYs real estate.
a Guardians shall have custody of the Responde�Cs ceal cstate; however, they shall have no
power to rent or sell said real estate soonec than six (6) months afrer the date of this O�der.
d. During the six (6) months following the date of this Order, Guudians shall pemiit
Respondent to have access ro her home, tiom time [o timc, as she may request, for the purpose of
collecting her clothing and othcr personal property.

e. Guardians shall make such periodic dcposits, not less often than monthly, o
Respondent's funds inm the RespondenYs personal cash account xt the Bridges as may be
necessary to maintain a balance the�ein of two hundred ($200) dollars, which shall be available
to the Rcspondent as"pockct money" for he� incidental purohases and outings.
£ Respondent shall be permitted to cetain a savings account with no debit card, a�d
Guardians shall deposit into such account any net income of thc Respondent remaini�g each
month after payment of RespondenPs bills and replenishment of Respondent's personal cash
aceount at the Bridges.
g. Guardians shall have custody of the Respondent`s automobile with authority to sell, and
authority to direct attomey Knight to withdraw the RespondenYs appeal of the suspension of her
drivee's license.
h. Gaardians shall have authoriry to rep�esent the Respondent io establishing her right to the
ceceipt of VA and Army retirement benefit payments, and to determine the method of payment
thereof.
i. The Limited Guardianship of the Respondent's person and estate s-hall continue on the
foregoing terms, until further Order of this Court.
