ORDER OF COURT
AND NOW, this day of , 2015, upon
consideration of the Petition by Cumberland County Aging & Community Services for
Appointment of Plenary Guardians of the Person and Estate of Ann Jenkins and the hearing held
on the Petition before this Court, the Court finds, based upon clear and convincing evidence, that
Ann Jenkins is an incapacitated person in that she suffers from mental impairment that prevents
her from receiving and evaluating information effectively and prevents her from formulating and
communicating decisions to such a significant extend that she is unable to manage her financial
affairs and meet the essential requirements for her physical health and safety on a daily basis.
Accordingly, Keystone Guardianship Services is hereby appointed as plenary guardians of the
person and estate of Ann Jenkins.
Keystone Guardianship Services shall serve without bond and is permitted to receive
compensation for its services as plenary guardians in the amount of$100 per month or ten
