PETITION TO AUTHORIZE THE DISTRIBUTION OF PROPERTY
BELONGING TO AND RELOCATION OF JOAN ANN RENZO
TO THE HONORABLE,THE NDGES OF SAID COURT:

AND NOW, this��day of � , 2015, comes the Petitioner,
Lauren A. Renzo, Guardian of the Estate and Person of Joan Ann Renzo, by and through her
counsel, Jessica F. Greene of Keystone Elder Law P.C., and files the following in support of
this Petition to Authorize the Distribution of Property Belonging to and Relocation of Joan
Ann Renzo.
RE:NRISDICTION
1. Jurisdiction lies in the Orphans' Court Division under 20 Pa. C. S. §711(10), which
states that the administration and distribution of the real and personal property of the
estates of incapacitated persons is to be exercised through the Orphans' Court Division.
RE:PREVIOUS ADJUDICATION
2. Previously, hereto, upon petition and hearing, Joan Ann Renzo was adjudicated an
incapacitated person by an Order dated the thirteenth day of June, 2014, signed by the

Honorable Judge Masland. By same Order, Lauren A. Renzo was appointed Plenary
Guardian of the Estate and of the Person of Joan Ann Renzo.
RE: PETITIONER
3. Petitioner, Lauren A. Renzo, residing at 107 C South Chestnut Street, Mechanicsburg,
Pennsylvania, 17055, is an adult individual and is the natural born daughter of Joan
Ann Renzo, an adjudicated incapacitated person. Petitioner is also the Plenary Guardian
of the said Joan Ann Renzo. Said Petitioner is a United States citizen and is able to
speak, read and write in the English language.
RE: INVENTORY
4. The inventory was filed on the 16th day of September, 2014.
5. The inventory detailed Joan Ann Renzo's personal property totaling approximately two
hundred five thousand two hundred ninety-five ($205,295.00) dollars and real property
with a tax assessed value of one hundred seventy six thousand seven hundred
($176,700.00) dollars.
RE: JOAN ANN RENZO
6. Joan Ann Renzo is an adult individual eighty-one (81) years of age, suffering from
Alzheimer's disease, which totally impairs her capacity to receive and evaluate
information effectively, and to make and communicate decisions concerning
management of her financial affairs, or to meet essential requirements for her physical
health and safety.
7. Joan Ann Renzo does not have an occupation, nor does she have any dependents.
8. Joan Ann Renzo does not have any creditors known to Petitioner.
9. Joan Ann Renzo's total current monthly income is one thousand eight hundred ninety-
two ($1,892.00) dollars from social security.
10. Currently, Joan Ann Renzo's assets consist of real estate ("family home") located at 6
Cedar Road, Mechanicsburg, Pennsylvania 17055, with a tax assessed value of one
hundred seventy six thousand seven hundred ($176,700.00) dollars, two vehicles that

total approximately four thousand one hundred ($4,100.00) dollars, and liquid assets
totaling approximately one hundred forty-two thousand ($142,000.00) dollars.
RE: TRANSFER OF JOAN ANN RENZO 1NT0 SKILLED NURSING FACILITY
1 l. Joan Ann Renzo resides at 6 Cedar Road, Mechanicsburg, Pennsylvania, 17055;
however, she is currently receiving care from Green Ridge Village located at 210 Big
Spring Road,Newville, Pennsylvania 17241.
12. She previously received respite care on an emergency basis from VibraLife, however
due to her increasing needs and requiring skilled nursing care; it became unsafe for her
to remain living in her home and at VibraLife. See Exhibit A.
13. Petitioner herein seeks permission to permanently place Joan Ann Renzo into the
skilled nursing facility, referenced above, for the care that Joan Ann Renzo requires.
RE: USE OF PRINCIPAL FUNDS FOR SKILLED NURSING CARE
14. As stated above, Joan Ann Renzo's current monthly income is one thousand eight
hundred and ninety-two ($1,892.00) dollars.
15. Joan Ann Renzo's cost of care at the facility ranges from nine thousand eight hundred
forty-one ($9,841.00) dollars to ten thousand eight hundred fifty-two ($10,852.00)
dollars per month, which does not include additional medical costs, such as for
prescription drugs, incontinence care, and services provided by a Physician; thus, Joan
Ann Renzo will need to use funds from her principal on a monthly basis to pay for her
costs at the facility.
16. No previous distributions have been requested of, allowed, or denied by the Court.
17. Petitioner herein seeks permission to use Joan Ann Renzo's principal funds in the
amount of ten thousand eight hundred fifty-two ($10,852.00) dollars (maximum) on a
monthly basis to pay for her care at the facility.
RE: USE OF PRINCIPAL FUNDS FOR REPAIR OF RESIDENCE
18. Petitioner herein seeks permission to use Joan Ann Renzo's principal funds to pay the
Borough of Mechanicsburg two thousand three hundred ninety-nine ($2,399.00) dollars

for sewer and sewage disposal connection to the family home. This connection is being
required by the Borough of Mechanicsburg. See Exhibit B.
19. Related to the above connection, permission is sought to pay Macmor Construction to
complete the connection in to the family home far the price of six thousand two
hundred forty-five ($6,245.00) dollars. See Exhibit C.
20. In addition, the family home requires specific repairs in order to prepare the home for
sale; thus, Petitioner requests permission to pay Joseph A. Renzo Construction in the
amount of three thousand five hundred forty dollars and sixty cents ($3,540.60) from
Joan Ann Renzo's principal funds. See Exhibit D
21. For the above repairs, Erie Insurance (homeowners) has provided three thousand four
hundred sixty-three dollars and fifty-four cents ($3,463.54)to Joan Ann Renzo and said
amount was added to her principal funds. See Exhibit E.
RE: PREPAID FLJNERAL
22. Petitioner herein seeks permission to prepay a funeral in the amount of twelve thousand
($12,000.00) dollars for the benefit of Joan Ann Renzo.
23. This amount is the average cost of a funeral, plus 25% for any extraordinary costs, in
Cumberland County (as evidenced by the attached Exhibit F), which is where the
Renzo family intends to bury Joan Ann Renzo, specifically, in a cemetery plot adjacent
to the plot of her late husband, Frank Renzo.
RE: REIMBURSEMENT OF PETITIONER
24. Because of her condition related to Alzheimer's disease, by the time that the Renzo
family sought legal assistance, Joan Ann Renzo lacked capacity to appoint an agent as
her durable financial Power of Attorney (POA) and a petition for guardianship had to
be filed.
25. As a result, Joan Ann Renzo's funds were unavailable during the time that the
guardianship was pending.
26. On multiple occasions during the above mentioned time period, Petitioner used her
personal funds to pay for care expenses for Joan Ann Renzo.

27. In addition, Petitioner used her personal funds to pay for the minimum necessary
expenses to responsibly maintain Joan Ann Renzo's real property.
28. Petitioner herein seeks permission to reimburse herself in the amount of one thousand
five hundred ninety-five dollars and seventeen cents ($1,595.17), from Joan Ann
Renzo's assets, for documented expenses paid from her personal funds for the personal
care, specifically clothing, groceries, co-pays, supplements prescribed by her
integrative doctor and other personal care items, of Joan Ann Renzo and far necessary
expenses to responsibly maintain her real property. A detailed spreadsheet with receipts
can be provided if necessary.
RE: GUARDIAN'S FEE
29. Petitioner herein seeks permission to distribute the amount of two thousand forty-six
dollars and twenty-five cents ($2,046.25) for her Guardianship services and mileage
related to the adjudicated incapacitated person as detailed in Exhibit G.
RE: ATTORNEY'S FEES
30. Petitioner herein seeks permission to distribute the amount of five thousand three
hundred fifty ($5,350.00) dollars to Keystone Elder Law P.C., located at
555 Gettysburg Pike, Suite G100, Mechanicsburg, Pennsylvania 17055, for attorney's
fees related to this petition and other legal matters concerning the adjudicated
incapacitated person and her estate.
WHEREFORE, Petitioner respectfully requests that your Honorable Court grant the
following relief:
Issue a Court order that authorizes Lauren A. Renzo, as Guardian of the Estate and
Person of Joan Ann Renzo,to do the following:
1. Permanently place Joan Ann Renzo in Green Ridge Village's skilled nursing care
and use principal funds to pay for said care monthly.
2. Use Joan Ann Renzo's principal funds in the amount of twelve thousand, one
hundred eighty-four dollars and sixty cents ($12,184.60) to repair Joan Ann
Renzo's real property.

3. In accordance with 20 Pa. C.S. § 5537,authorize the prepayment of funeral costs,in
the amount of twelve thousand ($12,000.00) dollars, for the benefit of Joan Ann
Renzo.
4. Use Joan Ann Renzo's principal funds to reimburse Lauren A. Renzo in the amount
of one thousand five hundred ninety five dollars and seventeen cents ($1,595.17)
for dacumented expenses paid from her personal funds for the care of Joan Ann
Renzo and for necessary expenses to responsibly maintain her real property.
5. Distribute the amount of two thousand forty-six dollars and twenty-five cents
($2,046.25) to Petitioner for the services she has provided the adjudicated
incapacitated person as Guardian.
6. Distribute the amount of five thousand three hundred fifty ($5,350.00) dollars for
attomey's fees to Keystone Elder Law P.C.

This is to inform you that the Borough is advertising far bids for the Cedar Road Sanitary
Sewer Extension. Bid op�ning is scheduled for July 10,with the intention to�eview and
possibly award ihe contract at the Borough Council meeting the following week.
Ke,epiu� it�simple; it usually take a month or two to do ali the paperwork (insnrance,
bonding,etc.)before Notice to Proceed is issued. After that occurs,the contractor wauld
have 90 days to complete the pmject. Residents must conaect to the public sewer within
45 days of Notice to Connect. Tliis will not�}tappen until the project is certified complete.

This is to inform you that construction of the sanitary sewer line to provide service to a section uf
Cedar Road is scheduled to begin on November 10, 2014. MacMor Construction of Dillsburg
was awarded the bid of.$88,3 i 8.80 and will have 40 days to complete the job. although they are
expecting to�do it in 3 or 4 weeks. Bryan Golden is the owner of MacMor ConstruMion. Their
office number is 432-7790, and Bryan's celt is 317-8288. Jerry Golden will atso be on site at
times. His celi number is 645-1407.
Conswction will begin on Market Street at the manhole south of the Cedar Road intersection
ar�d proceed onto Cedar Road. Construction witl be overseen by.the Borough's Codes and
Zoning Officer, Roger Ciecierski (691-3310 ext l03) and the Sewer Department Collection
System Operator,Greg Reinecker(691-3320 ext.5).
The Contractor will be dmpping off a stakz for you to iadicate where you would like your lateral
located. Please place it where it would best serve your existing plumbing exiting your house to
your septic system.
After constniction is completed, property ovsmers will be sent a Notice to Connect. Property
