This petition is filed to request the appointment of a guardian for the estate of a minor ffsuant
to section 5111 of the Probate, Estates and Fiduciaries Code, 20 Pa.C.S. §518: -;0

The Petition of Jamie A. Tyson respectfully states that:
1. The name of the minor for whose estate a guardian is requested is Willi"-H-T* ys2& Jr,-,z:

The minor is 13 years of age and was born on January 26, 2002. The mj'-fior resident
304 East Main Street, Shiremanstown, PA 17011.
2. The Petitioner is the natural mother of the minor is Jamie A. Tyson, who resides at 304
East Main Street, Shiremanstown,PA 17011.
3. The natural father of the minor is William H. Tyson, Sr., deceased on March 12, 2015.
4. Father did not appoint a guardian of the estate of the minor.
5. This petition is filed by the Mother of William H. Tyson, Jr., a minor, Jamie A. Tyson
whose consent is attached.
6. The minor has no guardian of his estate and the appointment of a guardian for his estate
is necessary because the minor is one of the intended beneficiaries of the proceeds from
the William H. Tyson, Sr. estate docketed at 21-15-0521, in the total amount of
$5,607.06, which are now payable to him.
7. Except as stated below, any proposed individual guardian is 18 years of age or older, has
no interest that is adverse to that of the minor, and is not a surety, or an employee of a
corporate fiduciary or surety, of an estate in which the minor has an interest, and any
proposed corporate guardian is authorized to act as a fiduciary in the Commonwealth,has
no interest that is adverse to that of the minor, and is not a fiduciary or surety of an estate
in which the minor has an interest. The proposed individual guardian is the executrix of
the estate of William H. Tyson, Sr., who was her Husband.
8. The following is an itemization of the assets of the estate of the minor and their location,
approximate value, and income, if any: The minor has no assets.
9. Notice to the United States Department of Veterans Affairs is not required.
WHEREFORE, your petitioner requests that she be appointed as guardian of the minor's
estate.

CONSENT OF PROPOSED GUARDIAN
1, Jamie A. Tyson, consent to act as guardian of the estate of William H. Tyson, Jr., a
minor.
I reside at 304 East Main Street, Shiremanstown, Cumberland County, PA 17011.
I am a citizen of the United States and can speak, read, and write the English language.
I have no interest adverse to the minor.
It is not my intention as the guardian to apply for an allowance for the support or
education of the minor during minority.
