Appointment of a guardian of the estate is needed to arrange for the application of the alleged incompetent's assets to the cost of her care and maintenance. 7. The minimal amount of the estate of the alleged incompetent is such that your petitioner requests that she be excused from filing inventory, bond or accounting so that the full amount may be applied to her care and maintenance. ORPHANS' COURT DIVISION NO. 2 1- 78- AN ALLEGED I NCmlPETENT PETITION FOR APPOINTMENT OF A GUARDIAN OF THE PERSON AND OF THE ESTATE TO TilE HONORABLE, THE JUDGES OF SAID COURT: The petition of Patricia Shively, daughter of the alleged incompetent, respectfully represents that: I. Your petitioner is Patricia Shively, who resides at R. D. 7, Box 131, Carlisle, (South Dickinson Township) Cumberland County, Pennsylvania. 2. The alleged incompetent is Hazel Leahy, who formerly resided at Meadowood Apartments, Allendale Road, Mechanicsburg, Cumberland County, Pennsylvania, and who, since March 27, 1978, has been residing with your petitioner at the address stated in paragraph I. 3. The alleged incompetent is 74 years of age, a widow, and the persons who would be entitled to share in her estate if she died intestate are: Patricia Shively, daughter, R.D.7, Box 131, Carlisle, PA. James Leahy, son, York Haven, PA. 4. The entire estate of the alleged incompetent consists of a 1974 Hornet automobile, valued at $1500.00, a checking account at Farmers Trust Company having a present balance of $ 262.86 , and a savings account at Farmers Trust Company None having a present balance of $ 5. Appointment of a guardian of the person is needed to arrange for the alleged incompetent's admission to Claremont Farms which is needed because of her hallucinations, refusal to eat properly, to take her medications, and to make responsible decisions concerning her person. lI;E~ 1D4 fAGE '782 .
