AND NOW, this 2nd day of September, 2015, upon consideretion of the
Amended Petition for Probate and Grant of Letters and an amended death certificate for
the decedent, Constantine T. Sideris, filed by James C. Sideris, IT IS DECREED that the
probate record shall be amended to reflect the correct Social Security Number. A new
Certificate of Grant of Letters shall be issued reflecting the correct Social Security
Number. Notification of this change has been sent to Pennsylvania Department of
Revenue, Inherilance Tax Division.
