Linda S. Hanson and C. Fred Uniatowski, by and through their counsel, Jessica F. Greene of
Keystone Elder Law P.C., and file the following in support of this Petition to Accept
Guardianship Transferred to this Commonwealth from the State of Delaware.
I. Ye[itioners, Linda S. Ilanson, curcently residing at 9426 Mackenzie Circle Court,
Sc Louis, Missouri 63123 and C. Fred Uniatowski, curcently residing at 5846 Waggoner's Gap
Road, Landisburg, Peunsylvaoia l7040, are the children and Plenary Guazdians of Mary C.
Uniatowski, the adjudicated iucapacitated person.
2. The adjudicated incapacitated persoo, Mary C. Uniatowski, is nn adult i�dividual
91 years of age, having been born on December 14, 1923.
3. The adjudicated incapacitated person is a widow and a resident of Cumbeiln�d
CounTy, Pennsylva�ia. She has been living at Thomwald Home with an address of 442 Walnut
6ottom Road, Carlisle, Cumbedand County, Pennsylvania 19013 for rivo and a half years. She
is not a patient of a mental hospital.

1. Petitioners, Linda S. Hanson, currently residing at 9426 Mackenzie Circle Court,
St. Louis, Missouri 63123 and C. Fred Uniatowski, currently residing at 5846 Waggonec's Gap
Road, Landisburg, Pennsylvania U04Q aze the children a�d Plenary Guardians of Mary C.
U�ia[owski, thc adjudicated incapacitated person.
2. The adjudicated incapacitated person, Mary C. Unia[owski, is an adult individual
91 years of age, having been bom on Uccember 14, 1923.
3. The adjudicated ineapacitated pe[son is a widow and a residen[ of Cumbedaod
County, Pennsylvania. She has been living at Thomwald Home with an address of 442 Walrmt
Bottom Road, Carlisle, Cumberland County, Pcnnsylvania 17013 for two and a half years. She
is not a patie�t oIa mental hospital.
4. The following individuals are the adjudicated incapacita[ed person's next of kin:

5. To the best knowledge, information and belief of Petitioners, the Gross Estate of
the adjudicated incapacitated person consis[s of two M&T Bazilc accou�ts that total
approximately five thousand ($5,000.00) dollars. Petitioners have filed theii annua] �eports
explaining the adjudicated persods assets as required by Delaware law.
6. To the best knowledge, information and belief of Petitioners, the income from all
sources of[he adjudicated incapacitated person is:
a. Social Security - $1,080.00
b. VA stipend - $95.00
7. Her total mo�thly i�come, not including unlrnown interest i�come, is believed to
be approximately one thousand one hu�d�ed sevenry-five ($1,175.00) dollars.
8. 'Che adjudicated incapacitated person was never a member of the Armed Services
of the United States, though her deceased h�sband was a WWII combat veteran and she is
currently receiving benefits from the United States Veterans Administration.
9. In addition, she is currendy receivi�g Medical Assistaoce for the mst of her
skilled nursing care.
10. The name and address of Ihc i�dividual or entiTy providing resideutial sewices to
the adjudicated incapacitated person is as follows: 7�homwald Home, 442 Walnut Bottom
Road, Carlisle, Cumbcrland Counry, Pennsylvania 17013.
l l. The following en[ities oe individuals are also preseotly pcoviding scrvices [o the
adjadicated incapaeitated person: none.
12. The Courl of Chancery of the State of Delaware has had jurisdiction previously
a�d determined that Mary C. Uniatowski is a disabled pe�son and granted guacdianship ro the
petitioners herein. The certitied copy of Delawaze's provisional order of transfer is attached.
13. The physical condition and limitations of the adjudicated incapacitated person is
as follows: Glaucoma in her left eye.
14. The adjudicated incapacitated person suffers from the following
eognitive/psychological infirmities: Dementia, severe eognitive impairment stated by hec
physician in April of 20I5.
15. A Guazdian is sought over [he following specific azeas of incapaciry: Plenary
Guardian of[he Person and Estate.
16. The adjudicated incapacitated person does not have a rela[ionship with an
a[torney at law who is expected to represent her in this matter, and given he� placeme�t in a
skilled nursing facility and state of confusion, is unlikely to be able to engage an attomey on her
own.
WHEREFORE, Petitioners pray this Honorable Court to issue an oider accepting the
Guazdians named in Delaware as Guazdians in this Commonwealth and accepti�g the
determination of incapacity and appointment of said Guardians without a hearing because the
adjudicated incapacitated person is bcd bound in a skilled nursing faciliTy and one of the
Guardians lives in Missouri.

1T IS HEREBY ORDERED that:
1) The guardianship of the above named disabled person is hereby
provisionaily transferred to the State of Pennsylvania.
2) The Petitioners shall file a petition for guardianship or transfer of
guardianship in the State of Pennsylvania.
3) Once a Court in the State of Peonsylvania grants a provisional
ocder accepting jurisdiction oPthe above guardiaoship, the
Petitioners shall file a copy of that order with the Court of
Chancery.
4) After all ofthe requirements in 12 DeI.C. § 39A-30L (� are
satisfied, the Court may issue a final order confirming the transfer
and terminating the guardianship i� the State of Delaware.
