James Polillo, born August 1, 2013 shall be amended so that heretofore henceforth that the
statement"Father's information not given" shall be removed from the child's Birth Certificate
and Jonathan Lee Polillo shall hereinafter be listed on the Birth Certificate as the father of the
subject minor child.
