PA 1701 I, a beneficiary in thc abovc-captioned Estate, heceby affinn and acknowledgc
that:
A. [ am a nephew of the decedent, Kichard U. Ri�esmith, Sr. who died on
January 5, 2015; and
B. I am the sole bencficiary of the Estate of Richard D. Rinesmith, Sr. in
accordance with the decedent's Last Will and Testament dated October 28, 2013 a�d
admitted probate on January I5, 2015 to the above-referenced docket in Cumbedand
Cou�ty. Pennsylvania and
C 1 am familiar with the assets and expenses of the Estate of Richard D.
Rinesmith, Sc including financial information conceming the value of all assets titled
Richard D. Rinesmith, Sr, decedent; and
