The Petitioner desires to change her name from Kacie Lynette Browand to Kacie Lynette Morrell pursuant to 54 Pa. C.S.A. ~701. 3. The Petitioner's reason for desiring this change of name is: A. Petitioner feels a strong connection to her Mother's side of the family and desires to have her name changed to her Mother's maiden name. B. By reason of her Father's conduct, Petitioner has been estranged from her Father and has had very little contact with him for the last four years. As a result, she no longer feels a connection to him and no longer wants to be known by his surname. 4. Petitioner requests this Court: A. To fix a hearing on this Petition; and B. To direct that notice be given of the filing of this Petition and the hearing.
