PETITION FOR CHANGE OF NAME
Petitioner, BARBARA DALE LEITZEL, by her undersigned attorney, requests that this
Court order a change of Petitioner's name pursuant to 54 Pa. Cons. Stat. pOl et seq. and in support
thereof makes the following averments:
I. Petitioner is an adult individual residing at 2750 Spring Hill Lane, Enola,
Cumberland County, Pennsylvania.
2. Petitioner was born Barbara Dale Stout on August 27, 1950.
3. Petitioner's name changed upon her marriage on January 17, 1969. Her name became
Barbara Dale Crary.
4. Petitioner subsequently was divorced.
5. Petitioner's name changed again upon her second marriage on September 28,1983
to Ronald Leitzel. Her name became Barbara Dale Leitzel.
6. Petitioner is now desirous of assuming her maiden name for use as her middle name.
7. Petitioner has resided at 2750 Spring Hill Lane, Enola, Cumberland County,
Pennsylvania, during the five years preceding the filing of this Petition.
8. There are no judgments or decrees of like character of record against the Petitioner.
WHEREFORE, Petitioner respectfully requests that this Honorable Court:
a. fix a hearing on this Petition;
b, direct that notice be given of this Petition and hearing; and
.
c. enter a decree changing Petitioner's name from BARBARA DALE
LEITZEL to BARBARA STOUT LEITZEL.

ORDER OF COURT
AND NOW, this 31st day of May, 2005, upon consideration of the attached
Petition for Change of Name, a hearing is scheduled for Thursday, August 4, 2005, at
9:30 a.m., in Courtroom No.1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, S6 (see 54 Pa. C.S. S701 [Historical and
