Your 'Petitioner is Luther L. Delp, who is a competent adult res~ding at,1016 Harris-
burg Pike, Carlisle, Cumberland County, Pennsylvania 17013, and 'who is the 'son and
only child of ~azel E. Delp.

Hazel' E. Delp', the alleged' incompetent is 82 years of age, is widowed and presently
resides at the Swaim Nursing Home in Newville, Cumberland County, Pennsylvania.

Hazel' E. Delp was widowed on October 20, 1977 when her husband Norris Henry Delp
of ~43 Cavalry Street, North Middleton Township, Carlisle, Cumberland County,
Pennsylvania, died.
Hazel E. Delp's next of kin is the Petitioner.
