PETITIONER'S INTERROGATORIES
TO ALLEGED INCOMPETENT
Petitioner, GENIEVE CLOUSE, by her counsel, SAIDIS &
GUIDO, pursuant to the provisions of The Pennsylvania Rules of
Civil Procedure, demands that the alleged incompetent answer
the following interrogatories, under oath, within twenty (20) days
after service thereof upon the alleged incompetent. If additional
space is required to fully answer the interrogatories, please
attach additional pages.
1. What are the names, addresses and professions of
all witnesses the alleged incompetent intends to call as
witnesses on his behalf at the hearing to determine his
competency?

2. What are the names and addresses of all physicians,
neurologists, surgeons or other doctors who have examined,
attended or treated the alleged incompetent or performed tests
upon him or have diagnosed his condition or have been 1n con-
sultation concerning his condition since the accident which
occurred on or about March 25, 1976?

3. As to each person named in the alleged incompetent's
answer to interrogatory number 2, please state:
(a) What service he rendered to the alleged
incompe ten t;
(b) When or over what period he rendered
said service;
(c) What, if any, treatment, medical supplies
or medication he prescribed for the alleged
incompeten t.
(d) What, if any, tests, x-rays, or other studies
he ordered for the alleged incompetent
and the dates, if any, said tests were
conducted.

4. What are the names and addresses of all hospitals,
emergency rooms or other institutions where the alleged in-
competent has been treated or observed concerning his condition
arising from the accident which occurred on or about March 25,
1976?
