IN THE ESTATE OF CINDY L. CLOUSE, DECEASED
ORDER OF COURT
AND NOW, tZ~vf-I, 1978, at 1/.!:7/ o'clock, e.,
E.D.S.T., upon consideration of the foregoing Petition and on
motion of Tom H. Bietsch, Esq., attorney for the Petitioners, the
proceeds of The Jefferson Standard Life Insurance Policy No.
3002279 and Metropolitan Life Insurance Company Group Policy No.
5534-G, Certificate No. 123319, shall be paid to Dauphin Deposit
Bank and Trust Company, Carlisle, Pennsylvania, for deposit in a
savings account in the name of Michelle Elizabeth Clouse, born
April 8, 1973, the beneficiary of said life insurance policies;
said savings account to be marked so that no withdrawals may be
made therefrom without order of court, and to be paid to said
minor child upon reaching her majority.

IN THE ESTATE OF CINDY L. CLOUSE, DECEASED
PETITION FOR PAYMENT OF INSURANCE PROCEEDS
TO THE HONORABLE JUDGES OF SAID COURT:
The Petition of Charles R. Clouse and Mary L. Clouse,
respectfully represent:
1. That Cindy L. Clouse, late of Lower Mifflin Township,
Cumberland County, Pennsylvania, died on December 26, 1977.
2. That decedent was survived by one child, a minor daughter
Michelle Elizabeth Clouse, born April 8, 1973; and her parents,
your Petitioners, as her sole heirs.
3. That Michelle Elizabeth Clouse is presently residing
with and being supported and maintained by your Petitioners.
4. That on February 15, 1978, Letters of Administration in
decedent's estate were issued to your Petitioners by the Register
of wills for Cumberland County.
5. That decedent, at the time of her death, was the owner of
The Jefferson Standard Lifp. Insurance Policy No. 3002279, insuring
her life, and naming Michelle Elizabeth Clouse as beneficiary.
The amount of said policy proceeds is approximately Five Thousand
One Hundred Dollars ($5,100.00).
6. That decedent, at the time of her death, was further
owner of Metropolitan Life Insurance Company Life Insurance Policy
Group No. 5534-G, Certificate No. 123319, insuring her life, and
naming Michelle Elizabeth Clouse, as beneficiary. The amount of
said policy proceeds is approximately Two Thousand Dollars
($2,000.00).
7. That the estate of decedent appears to be insolvent so
that nimimal proceeds, if any, will be due Michelle Elizabeth
Clouse as intestate heir in the settlement of decedent's estate.
8. That your Petitioners desire the proceeds of both said
life insurance policies be deposited in a savings account in

Dauphin Deposit Bank and Trust Company, Carlisle, Pennsylvania,
to be paid to Michelle Elizabeth Clouse when she reaches her
majority.
9. That Dauphin Deposit Bank and Trust Company, Carlisle,
Pennsylvania, has no interest in this matter adverse to the
interest of the aforesaid minor, Michelle Elizabeth Clouse.
WHEREFORE, your Petitioners pray your Honorable Court direct
that the insurance proceeds from the aforesaid life insurance
policies be paid to Dauphin Deposit Bank and Trust Company,
Carlisle, Pennsylvania, to be deposited in a savings account for
Michelle Elizabeth Clouse, said account to be marked in such
fashion that no withdrawals shall be made therefrom without order
of court prior to said child reaching her majority; and the'
balance in said account to be paid to said child when she reaches
her majority.

according to law, depose and state that the facts set forth in
the foregoing Petition are true and correct to the best of their
