AND NOW, comes the claimant, TOWNSHIP OF HAMPDEN, by the Township Manager, who
files the following claim against Stephen E. & Dawn A. Earl and that certain tract of
land including improvements thereon, owned by them and described as follows:

For sewer/trash services rendered from 10/01/04 to 4/30/05. Said services were
rendered and charged pursuant to ordinance. Attached hereto is a statement of the kind
and character of the services so rendered and the price charged therefor, said statement
being a true capy of the owner's account with Hampden Township which sum of $379.99 was
duly assessed against the hereinabove-named owner and the premises on 4/30/05 and for
which sum, with interest, penalties and costs, a lien is claimed against the above-described
premises in accordance with the Act of May 16, 1923, P.L. 207 (53 P.S. Section 7101,
et seq.), as amended and supplemented, and the Act of June 19, 2001, P.L. 287 (53 P.S.
Section 5601, et seq), as amended and supplemented.
