PETITION FOR NAME CHANGE
AND NOW comes Petitioner, Steven Eugene Burke, by and through his attorney,
Richard R. Gan, Esquire and makes the following averments in support of this Petition
for Name Change pursuant to 54 Pa C.S. 702 pertaining to judicial change of name.
I. Steven Eugene Burke is an adult with a date of birth of August 15, 1963. Please
see Birth Certificate attached hereto, marked Exhibit A and incOlporated by
reference.
2. The petitioner resides at 7 N Corporation Street, Apt 1, Newville, P A 17241.
3. The petitioners social security number is 206 54 3719. Please see a copy of
Petitioners social security card which is attached hereto and marked exhibit B.
4. The petitioner's birth name is Steven Eugene Junkins and he wishes to have his
birth name restored.
5. The petitioner wishes to further be known as Steven Eugene Junkins due to the
impending birth of his son.
6. The legal standard for a Petition for Name change is that it is not for fruadulent
purposes and in the best interest of the petitioner.
7. It is further the intent of the statute to make certain that there is no social stigma
and that the use of a particular name in the community is neutral and therefore not
an issue.
8. There is no fraudulent purpose behind this request for a name change.
9. Petitioner has been fingerprinted in accordance with the judicial change of name
requirements at the Pennsylvania State Police and his fingerprint card is attached
hereto as Exhibit C.
WHEREFORE, Petitioner prays that a hearing be scheduled to address the issues
raised herein.
