PETITION FOR APPOINTMENT OF LEGAL GUARDIAN
, comes the Petitioner, Mary W. Tark, by her attorneys, Snelbaker,
Brenneman & Sp e, P.C., and avers the following:
1. T e Petitioner is Mary W. Tark, an adult individual residing at 1390 Simpson
Ferry Road, New Cumberland, Cumberland County, Pennsylvania 17070.
2. P .tioner is the paternal grandmother of Tyler Wayne Chancey, age 12, born
February 11, 199 .
3. P titioner's son, Ronald Wayne Chancey, father of Tyler Wayne Chancey, died as
a result of an aut mobile accident in Georgia on January 30, 2002.
4. Tier has resided with Petitioner and her husband since 1990, providing a stable
and permanent h me environment for Tyler.
5. Tier's natural mother, Evelyn Karen Rogers Smith has a last known address of
2011 Lentz Driv ,Blackshear, Georgia 31516. The mother has had very limited contact with
Tyler since birth In the child custody case docketed to No 02-894 Civil Action in the Court of
Common Pleas f Cumberland County, Pennsylvania by Order dated March 27, 2002, a copy of
which is attach hereto as Exhibit A, Petitioner was awarded sole legal and primary physical

died, is willing to pay $10,000 to Tyler Wayne Chancey's legal guardian.

e purpose of this Petition is to have Petitioner appointed as guardian of the
estate of Tyler W yne Chancey so the insurance company will pay the $10,000 and for other
purposes that ma arise as a result of Petitioner having sole legal custody and primary physical
custody of Tyler.
8. N other guardian has been appointed for the minor's estate.

17070 and am e ployed as Assistant Director of Nursing at Villa Teresa Nursing Home.
I am a ci zen of the United States and can speak, read and write the English language.
I have no interest adverse to the minor, Tyler Wayne Chancey.
It is not y intention to apply for an allowance for the support or education of the minor
during minority.
